Ventura Pelvic Physical Therapy is an out of network provider for all insurance companies.  All services $150 for 45 min evaluation and follow up treatments.  

Patients will be provided a superbill upon request to submit to their insurance company.  If the patient's insurance plan has out of network benefits the patient will be sent reimbursement from their insurance company directly.
